This page is intended to provide a single place in which to register problems with or submit complaints about any of the centrally available classrooms, as listed below, with respect to room or A/V equipment deficiencies. It is not intended to nor is it staffed to respond to emergencies, to deal with problems in classrooms that are not centrally available, or to trigger major building renovations.
In order to submit a complaint or register a problem please:
Select the classroom from the list below. (LSM has no responsibility for the state of other classrooms.)
Provide your e-mail address, name and department on the form below.
Identify the problem in the comments section and submit it.
LSM will respond to each submission either by forwarding it to the appropriate office, with a copy to you, or by responding directly to you (by e-mail, only).
